Health care professionals who provide you with care are required by law to maintain records about your health and any treatment or care you have received. These records help to provide you with the best possible healthcare.
We collect and hold data for the sole purpose of providing healthcare services to our patients. We keep your information in digital form and the records may include basic details about you, such as your name and address, however, they will also contain more sensitive information about your health.

Information this practice processes about you may include the following:

>> Any contact the practice has had with you, such as appointments, emails
>> Notes and reports about your health
>> Details about your treatment and care
>> Results of investigations
>> Relevant information to and from other health professionals
>> Payment information such as credit/debit card and health insurance provider
Our Lawful Basis for Processing Data
Urologists Ltd act as both controller and processor of personal information.
As controller, our processing activities include arranging appointments between data subjects (patients) and consultants for the purpose of consultation, examination, treatment, diagnostic and/or medical management services.
In arranging an appointment, the legal basis for processing personal data, such as a data subject’s name, contact details, date of birth, will be contractual. It will be an informal
contract between the data subject and us, Urologists Ltd, with the understanding that we will provide a service in exchange for payment. The data subject or their representative will be advised of this arrangement at the time of booking, as well as their rights in relation to processing their information.
As processor our processing activities include arranging appointments between third-party referrers acting on behalf of an individual data subject and our clinicians.
Where data is collected from third-parties, there will be a contractual agreement between
Urologist Ltd and the third-party.
Special Category Data
Special category data is personal data which the GDPR says is more sensitive, and so needs more protection. When processing special category data, we must identify both a lawful basis under Article 6 and a separate condition for processing special category data under Article 9.
In accordance with GDPR Article 9 section 2 (H) ‘processing is necessary for the purposes of preventive or occupational medicine, for the assessment of the working capacity of the employee, medical diagnosis, the provision of health or social care or treatment or the management of health or social care systems and services on the basis of Union or Member State law or pursuant to contract with a health professional and subject to the conditions and safeguards referred to in paragraph 3’.
